Placing 3.0 Forms Throughout the EMR

How to replace your custom 2.0 forms with 3.0 forms in the admission checklist, intakes, discharge planning, family portal, and treatment plan reviews.

ALL custom 2.0 forms need to be replaced with new 3.0 forms before we sunset Forms Manager 2.0. Our team will assist with rebuilding your existing forms, but you will need to place them throughout the system (Intakes, admission checklist, discharge summary, etc.).


In order to switch out forms in all areas of the EMR, follow these steps:

  1. Make sure all in-progress forms are signed/submitted. By signing a form, you lock it so that changes you make in settings won't affect it and you won't lose data entered into the form.

  2. Once all in-progress forms are signed, go down the list of these tabs in System Settings to replace old forms:



  3. In each tab, toggle custom 2.0 forms from "Yes" to "No" and add in the new 3.0 versions of them by selecting "Add Section" at the top. Reorder with drag and drop as needed, and save.

  4. Repeat steps 1-3 in all of your different locations (if applicable). Each location in your Alleva EMR has its own independent documentation, so each one needs to be converted over to 3.0.
