How to add addendums to Individual Notes.
The Notes Addendums feature in Alleva allows clinicians to add additional information to a note after it has been signed, without having to unlock or edit the original entry. Each addendum is timestamped and signed separately to maintain a clear and compliant documentation trail.

How to Add an Addendum to an Individual Note:
1. Go to the Notes screen within the client’s chart.
2. Find the completed and signed note to which you want to add an addendum and click the plus (+) icon in the Actions column next to the note.
TIP: Only notes that have been signed and completed will display the Add Addendum button.
4. Type your addendum text into the text box that appears.
5. Click Save and Sign, then enter your signature to confirm the addendum.
6. Verify that the addendum appears in the Addendums column with the count updated. Click the printer icon to view the full note, including the original content and all addendums.
Done! Each addendum is clearly labeled with the text, timestamp, the name of the person who added it, and their signature.
