How to create custom intake and note templates in Alleva Echo.

Alleva Echo allows admins to create custom templates for session notes, ensuring that AI-generated documentation is structured exactly how your organization needs it. These custom templates can be used for intake sessions, group sessions, and individual sessions.

How to Create a Custom Template in Alleva Echo:

1. Go to System Settings and click Templates from the left-hand menu.

2. Select Echo from the dropdown to open the Alleva Echo dashboard in a new tab.

Note: Only users with the Rehab Admin role will see this option.


3. Click the Add a Form button to create a new template.

4. Enter a template name and add labels, headings, and prompts to define each section of your form. Add additional labeled fields or subsections as needed to capture more content areas.

5. Use ChatGPT to help write high-quality AI prompts. Visit chat.openai.com and provide detailed instructions like: “Help me write a prompt for AI to summarize a biopsychosocial assessment focusing on diagnosis, previous treatment, and family history in 50–100 words using professional language.” After generating the prompt through chatgpt, paste it into the "Prompt" field in the Alleva Echo form.

6. Click Save for Self to keep the template private or Save for Group to share it with your team.


Done! Now when you use Echo in group sessions, individual sessions and intakes, you will be able to select the new template/form type when generating the note. Navigate back to System Settings > Templates > Echo at any time to make edits to templates.
